Please read these instructions before filling in the form.

* This is an order form, not an online shop. By filling in this form your order will be sent to YAM member Symor Skilbeck for processing.
* We are only able to accept payment by PayPal
* We rely upon you to add up the correct amount for the items you order and make that payment by PayPal when you submit the form.
* Orders received without payment will not be processed.
* There are several clothing choices and should you wish to order multiple items in different sizes you will need to complete a separate form
